I am sure that Andy will confirm but time is not, usually, added for throw ins. (Unlike penalties). According to the laws if the ref thought that 15 seconds had passed waiting for the throw then the ball need only be in play for 5 secs after the throw. It is my understanding that the whistle for full time can go even before the throw has been taken.

You might allow time for a throw-in (or other restart) if there is excessive time taken, which in lower level football usually means getting the ball back from over the hedge or someone's back garden(or one game I did in Waltham Abbey somewhere once, out of the river). Under these circumstances one would stop one's watch. In loftier echelons, I suppose if the crowd deliberately delayed returning the ball, as used to happen, you would add time, but now they use another ball.

 

Of course there is also the issue of time-wasting, which you could argue is a different issue.

 

Time is indeed added for a penalty kick: if the 90 minutes is up after the offence, the penalty kick must be completed and the game finishes as soon as the ball stops travelling forwards i.e. if the goalie saves it or it comes back off the post, the kick-taker cannot score with the rebound.
