Don't know about the most, but Clarence Seedorf is the only player to have won the Champions League with 3 different clubs, i.e. Ajax, Real Madrid and twice with AC Milan, so that's 4!!!

 

I believe Paolo Maldini has won 5 times and runner up 3 times, so that's 8.
